Form Contacts
A user can be a contact on a form based on the roles that their user has been assigned.
When a user creates a form, they will be be the default From Facility contact. However, they are able to select a different contact if they would like. The contact information (email and phone number) is pulled form the user's profile and will auto fill when the user is selected.
On a templated form, the contact information that is on the new form is pulled directly from the contact information on the first ever created form. You can then change this contact prior to submitting.
To prevent having to change the contact information on templated forms every month, you can create a new form and begin templating the new form with the corrected contact information forward every month. You can do so by clicking on the "New" button on the top of the screen.
